The reason why we ask for identification each and every time you arrive for an appointment is to make sure that you are matched with the correct clinical records. Even if you’re a regular patient to the practice you will still need to identify yourself each time you attend. It is important that we will be able to contact you and there are many reasons why this may happen. For example, to give investigation results, to send health reminders or to contact you in the event of an emergency. Another reason is in case there is an infectious disease outbreak where we may need to contact you for early intervention. It is very important to keep your details up to date at all times.
